The ANNUAL EMS SURVEY

Reporting Period: January 1 – December 31, 2007

The EMS survey provides the State with basic information about the Alaska EMS system. The data is used for local, regional and statewide planning and evaluation; improving training and testing; injury prevention efforts; and funding requests.  Both ground ambulance and first responder services are included in the survey. 

In the coming year we will be developing a web-based data reporting system which will be free to all services.  We are very excited about the project and look forward to working to working with you to transition to a paperless reporting system. 

The 2007 Survey is a modifiable PDF form, thanks to Emily McKenzie at Southern Region EMS Council.  Have an authorized person for your service fill it out online, and then print, sign, and fax, mail, or scan and email it to your EMS Region.  The deadline is April 1, 2008.  Below are addresses and fax numbers for your EMS Regional office.  You can confirm your EMS Region in the State EMS Directory.
